    Perceptions of campus climate : rural LGBTQ+ student experiences at a rural midwest university

    Rural lesbian, gay, bisexual, and transgender (LGBTQ+) high school students face harassment, lack connection to their schools, and have low college aspirations (Palmer, Kosciw, & Bartkiewicz, 2012). Studies focused on LGBTQ+ students in college do not address the needs of rural students, often painting a wide brush of campus experiences for all LGBTQ+ students. This study focused on LGBTQ+ college students from rural areas studying at a rural Midwestern university. The study included nine interviews, a document analysis, and a literature review exploring the experiences of rural LGBTQ+ students and campus climate at Midwest University. This qualitative study addressed the gap in literature concerning perceptions of campus climate for rural LGBTQ+ students in college and the impact of sexual identity development in college (Zubernis, Snyder, Mccoy, 2011). It is imperative institutions work to provide opportunities for LGBTQ+ students to connect with one another and offer a variety of inclusive events promoting the value of all students.Includes bibliographical reference

    Cognitive development Respiratory Tract Illness and Effects of eXposure (CORTEX) project: Data processing challenges in combining high spatial resolution pollution level data with individual level health and education data

    Background and Objectives: There is a lack of evidence of the adverse effects of air pollution and pollen on cognition for people with air quality-related health conditions. The CORTEX project combined routinely collected health and education data, high spatial resolution air pollution modelling, and daily pollen measurements for 18,241 pupils living in Cardiff, UK, between 2009 and 2015, to investigate the acute effects of air quality and respiratory conditions on education attainment. Datasets: Air pollutants PM2.5, PM10, NO2, and ozone levels were modelled for 157,361 home and school locations, anonymised into the Secure Anonymised Information Linkage (SAIL) Databank, and summarised into minimum, average and maximum readings for 4 daily time periods reflecting pupil home/school exposure. Adding a unique Residential Anonymised Linking Field (RALF) allowed linkage of pollution estimates to individual level data. Annual pollution datasets contained 369 columns and 472,083-rows, with one column per location, pollutant, daily time-period and day of year. Dataset transformation produced a 5 column, 3,446,205,900-row matrix per year. Methods and Conclusions: An algorithm using Structured Query Language (SQL) to manage data held within a relational database management system, was designed to reduce dimensionality from 24 billion to 18,241 rows of data. The algorithm calculated average means for each pollutant (PM2.5, PM10, NO2, and ozone levels) over the revision and examination periods, and summarised data into one row per pupil. The algorithm adjusted for weekends, school, and bank holidays, it calculated daily pollutant exposure for each pupil, and successfully linked 95% of pupil pollution exposures to their health and education data

    VCAM1/VLA4 interaction mediates Ly6Clow monocyte recruitment to the brain in a TNFR signaling dependent manner during fungal infection

    Partial funding for Open Access provided by the UMD Libraries' Open Access Publishing Fund.Monocytes exist in two major populations, termed Ly6C^hi and Ly6C^low monocytes. Compared to Ly6C^hi monocytes, less is known about Ly6C^low monocyte recruitment and mechanisms involved in the recruitment of this subset. Furthermore, the role of Ly6C^low monocytes during infections is largely unknown. Here, using intravital microscopy, we demonstrate that Ly6C^low monocytes are predominantly recruited to the brain vasculature following intravenous infection with Cryptococcus neoformans, a fungal pathogen causing meningoencephalitis. The recruitment depends primarily on the interaction of VCAM1 expressed on the brain endothelium with VLA4 expressed on Ly6C^low monocytes. Furthermore, TNFR signaling is essential for the recruitment through enhancing VLA4 expression on Ly6C^low monocytes. Interestingly, the recruited Ly6C^low monocytes internalized C. neoformans and carried the organism while crawling on and adhering to the luminal wall of brain vasculature and migrating to the brain parenchyma. Our study reveals a substantial recruitment of Ly6C^low monocytes to the brain and highlights important properties of this subset during infection.https://doi.org/10.1371/journal.ppat.100836

    Qualitative interview study exploring the patient experience of living with axial spondyloarthritis and fatigue : difficult, demanding and draining

    Objective: To explore patients’ lived experiences of axial spondyloarthritis (axSpA) and fatigue. Design: Interpretative phenomenological analysis (lived experience) was used as the study design. Analysis drew together codes with similar meaning to create superordinate and subordinate themes. Setting: Rheumatology departments in three National Health Service Foundation Trusts in the north, midlands and south of England. Participants: A purposive sample of seventeen axSpA patients were recruited. The age range was 22–72 years (median age 46), nine were male and eight, female. Results: A central concept of achieving balance was identified as the active process of integrating axSpA symptoms and fatigue into daily life, working with and not against their condition to lead a fulfilled life. This was conveyed through three superordinate themes: struggling to find energy, engaging in everyday life and persevering through difficulties. Struggling to find energy was the challenge of retaining enough stamina to do things in daily life. Engaging in everyday life highlighted dedication to being active and organised, learning through experience and acceptance of a changed way of being. Persevering through difficulties identified the physical and emotional effort required to keep moving forward and the importance of feeling supported. Conclusion: Achieving balance through finding energy, engaging and persevering everyday was fundamental to having the best possible life. The experience of energy emerged as a distinct but related component of fatigue. However, while energy could be maintained or replenished, fatigue was more difficult to overcome and required greater effort. Energy may be a useful indicator of an individual’s current state and ability to sustain activities that supports their well-being, such as exercise. Awareness of the elements of achieving balance in axSpA may enable patients and clinicians to work together to tailor treatments to individual patient need

    A New Performance Measurement System for Maternal and Child Health in the United States

    Abstract Objective The Title V Maternal and Child Health (MCH) Block Grant is the linchpin for US MCH services. The first national performance measures (NPMs) for MCH were instituted in 1997. Changing trends in MCH risk factors, outcomes, health services, data sources, and advances in scientific knowledge, in conjunction with budgetary constraints led the Maternal and Child Health Bureau (MCHB) to design a new performance measurement system. Methods A workgroup was formed to develop a new system. The following guiding principles were used: (1) Afford States more flexibility and reduce the overall reporting burden; (2) Improve accountability to better document Title V's impact; (3) Develop NPMs that encompass measures in: maternal and women's health, perinatal health, child health, children with special health care needs, adolescent health, and cross-cutting areas. Results A three-tiered performance measurement system was proposed with national outcome measures (NOMs), NPMs and evidence-based/informed strategy measures (ESMs). NOMs are the ultimate goals that MCHB and States are attempting to achieve. NPMs are measures, generally associated with processes or programs, shown to affect NOMs. ESMs are evidence-based or informed measures that each State Title V program develops to affect the NPMs. There are 15 NPMs from which States select eight, with at least one from each population area. MCHB will provide the data for the NOMs and NPMs, when possible. Conclusions The new performance measurement system increases the flexibility and reduces the reporting burden for States by allowing them to choose 8 NPMs to target, and increases accountability by having States develop actionable ESMs

    Are children who are treated for asthma and seasonal allergic rhinitis disadvantaged in their educational attainment when acutely exposed to air pollution and pollen? A feasibility study

    Background There is a lack of evidence of the adverse effects which air quality has on cognition for people with air quality-related health conditions, these are not widely documented in the literature. Educational attainment, as a proxy for cognition, may increase with improved air quality. Objectives Prepare individual and household level linked environmental and health data for analysis within an anonymised safe haven; analyse the linked dataset for our study investigating: Cognition, Respiratory Tract illness and Effects of eXposure (CORTEX). Methods Anonymised, routinely collected health and education data were linked with high spatial resolution pollution measurements and daily pollen measurements to provide repeated cross-sectional cohorts (2009-2015) on 18,241 pupils across the city of Cardiff, using the SAIL databank. A fully adjusted multilevel linear regression analysis examined associations between health status and/or air quality. Cohort, school and individual level confounders were controlled for. We hope that using individual-level multi-location daily exposure assessment will help to clarify the role of traffic and prevent potential community-level confounding. Combined effects of air quality on variation in educational attainment between those treated for asthma and/or Severe Allergic Rhinitis (SAR), and those not treated, was also investigated. Findings Asthma was not associated with exam performance (p=0.7). However, SAR was positively associated with exam performance (p<0.001). Exposure to air pollution was negatively associated with educational attainment regardless of health status. Conclusions Irrespective of health status, air quality was negatively associated with educational attainment. Treatment seeking behaviour may explain the positive association between SAR and educational attainment. For a more accurate reflection of health status, health outcomes not subject to treatment seeking behaviours, such as emergency hospital admission, should be investigated

    Analogy-Making as a Core Primitive in the Software Engineering Toolbox

    An analogy is an identification of structural similarities and correspondences between two objects. Computational models of analogy making have been studied extensively in the field of cognitive science to better understand high-level human cognition. For instance, Melanie Mitchell and Douglas Hofstadter sought to better understand high-level perception by developing the Copycat algorithm for completing analogies between letter sequences. In this paper, we argue that analogy making should be seen as a core primitive in software engineering. We motivate this argument by showing how complex software engineering problems such as program understanding and source-code transformation learning can be reduced to an instance of the analogy-making problem. We demonstrate this idea using Sifter, a new analogy-making algorithm suitable for software engineering applications that adapts and extends ideas from Copycat. In particular, Sifter reduces analogy-making to searching for a sequence of update rule applications. Sifter uses a novel representation for mathematical structures capable of effectively representing the wide variety of information embedded in software. We conclude by listing major areas of future work for Sifter and analogy-making in software engineering.Comment: Conference paper at SPLASH 'Onward!' 2020.     This paper addresses the empirical analysis of wholesale profit margins using data of the Dutch wholesale sector, 1986. At the heart of the analysis is the typical nature of wholesale production: wholesalers do not produce a tangible product, but offer a service capacity. This has an immediate impact on the identification, interprelation and measurement of determinants of profit variations. A model is set up to explain variations in wholesale profit margins, which is inspired by two widely applied approaches to industry pricing: the behavioural mark-up model and the marginalist price-cost model

    The BARRIERS scale -- the barriers to research utilization scale: A systematic review

    <p>Abstract</p> <p>Background</p> <p>A commonly recommended strategy for increasing research use in clinical practice is to identify barriers to change and then tailor interventions to overcome the identified barriers. In nursing, the BARRIERS scale has been used extensively to identify barriers to research utilization.</p> <p>Aim and objectives</p> <p>The aim of this systematic review was to examine the state of knowledge resulting from use of the BARRIERS scale and to make recommendations about future use of the scale. The following objectives were addressed: To examine how the scale has been modified, to examine its psychometric properties, to determine the main barriers (and whether they varied over time and geographic locations), and to identify associations between nurses' reported barriers and reported research use.</p> <p>Methods</p> <p>Medline (1991 to September 2009) and CINHAL (1991 to September 2009) were searched for published research, and ProQuest<sup>® </sup>digital dissertations were searched for unpublished dissertations using the BARRIERS scale. Inclusion criteria were: studies using the BARRIERS scale in its entirety and where the sample was nurses. Two authors independently assessed the study quality and extracted the data. Descriptive and inferential statistics were used.</p> <p>Results</p> <p>Sixty-three studies were included, with most using a cross-sectional design. Not one study used the scale for tailoring interventions to overcome identified barriers. The main barriers reported were related to the setting, and the presentation of research findings. Overall, identified barriers were consistent over time and across geographic locations, despite varying sample size, response rate, study setting, and assessment of study quality. Few studies reported associations between reported research use and perceptions of barriers to research utilization.</p> <p>Conclusions</p> <p>The BARRIERS scale is a nonspecific tool for identifying general barriers to research utilization. The scale is reliable as reflected in assessments of internal consistency. The validity of the scale, however, is doubtful. There is no evidence that it is a useful tool for planning implementation interventions. We recommend that no further descriptive studies using the BARRIERS scale be undertaken. Barriers need to be measured specific to the particular context of implementation and the intended evidence to be implemented.</p
